Our Services:

We provide compassionate, client-centered housing navigation and transition services designed to support individuals experiencing homelessness or housing instability.

Our approach focuses on meeting each client where they are, offering personalized guidance to secure safe, stable housing while addressing barriers that may stand in the way.

From conducting assessments and developing individualized housing plans to assisting with applications, documentation, and coordination with community resources, we are committed to creating a smooth and supportive path toward long-term housing stability.

We work collaboratively with healthcare providers, community partners, and support systems to ensure each client receives comprehensive care that promotes independence, dignity, and lasting success.

Our Programs

Through our partnership with Inland Empire Health Plan (IEHP), Thrive provides CalAIM Community Supports to help individuals secure and maintain stable housing. Services include Housing Deposits, Home Modifications, Housing Transition and Navigation Services, and Housing Tenancy and Sustaining Services. Participants must have IEHP, be enrolled in Medi-Cal, and meet eligibility guidelines to receive services.

  • Housing Transition and Navigation Services (HTNS) helps members find and secure stable housing. Services may include completing housing assessments, developing a housing support plan, searching for housing, assisting with housing applications, communicating with landlords, coordinating move-in activities, and connecting members to community resources. The goal is to help members transition from homelessness or unstable housing into permanent housing.

  • Housing Deposits are one-time financial supports that help members obtain housing. Covered items may include security deposits and other reasonable one-time move-in expenses necessary to secure safe housing. These services are intended to remove financial barriers to obtaining permanent housing.

    Restrictions and Limitations apply.

  • Housing Tenancy and Sustaining Services (HTSS) supports members in maintaining stable housing after move-in. Services focus on preventing eviction and promoting long-term housing stability. This may include tenant education, conflict resolution with landlords or neighbors, budgeting assistance, coaching on household responsibilities, assistance understanding lease agreements, and coordination with medical, behavioral health, or social service providers.

  • Environmental Accessibility Adaptations (EAAs also known as Home Modifications) are physical adaptations to a home that are necessary to ensure the health, welfare, and safety of the individual, or enable the individual to function with greater independence in the home, without which the member would require institutionalization.

    Home Modification services provide physical adaptations to a member’s home to improve accessibility, safety, independence, and functioning related to a medical condition or disability. Examples may include wheelchair ramps, grab bars, widened doorways, roll-in showers, stair lifts, or other medically necessary modifications that help the member remain safely in their home and avoid institutional care.
