We are a Medi-Cal (CalAIM) provider specializing in California Advancing and Innovating Medi-Cal (CalAIM) initiatives, which transform Medi-Cal by enhancing care coordination, integrating social services into healthcare, and improving outcomes for members. Our expertise lies in delivering Community Supports through the Housing Trio Services: Housing Transition and Navigation Services (HTNS), Housing Deposits (HD), and Housing Tenancy and Sustaining Services (HTSS). Our mission is to promote housing stability and self-sufficiency for Medi-Cal members experiencing or at risk of homelessness.

As an IEHP-contracted provider, we assist members facing homelessness or at risk of homelessness, while addressing related social needs. Eligible members may receive moving cost assistance—including security deposits and household essentials—through IEHP’s one-time, lifetime Housing Deposit benefit, pending approval.
